Pubmed Search Button Reduce the rate of new cases of end-stage renal disease (ESRD). (Objective 04-01)
Pubmed Search Button Reduce deaths from cardiovascular disease in persons with chronic kidney failure. (Objective 04-02)
Pubmed Search Button Increase the proportion of treated chronic kidney failure patients who have received counseling on nutrition, treatment choices, and cardiovascular care 12 months before the start of renal replacement therapy. (Objective 04-03)
